Managing Pain After Surgery

Cochrane authors wanted to find out how effective different methods are in reducing the amount and severity of shoulder pain following gynaecological keyhole surgery.
Background
Gynaecological keyhole surgery is a procedure where a surgeon uses a camera to see inside the lower abdomen to view the uterus , fallopian tubes and ovaries. They can also use special instruments to do tests or treat certain gynaecological conditions. This is a common procedure that about 250,000 women in the UK have each year. Up to 80% of these women may experience shoulder-tip pain , which may be very painful and lead to longer stays in hospital and even having to go back in to hospital.
During the laparoscopy, the surgeon puts gas into the patients abdomen . This inflates the abdomen so that the surgeon can see the organs in the abdominal cavity and can carry out surgery. It is possible that inflating the abdomen stimulates a nerve that runs from the top of the abdomen up to the shoulders and neck, which causes STP.
We looked at several ways surgeons try to reduce STP: putting local anaesthetic directly into the abdominal cavity or diaphragm using warmed carbon dioxide, sometimes with moisture added to it during surgery removing gas from the abdominal cavity with drains replacing gas with fluid or forcing gas out of the abdominal cavity at the end of the procedure by increasing the pressure at which patients were made to breathe whilst still under anaesthetic .

But the left over traces of carbon dioxide can cause abdominal pain or back pain or right side pain after the surgery. Subjects And Study Design
This is a prospective randomized controlled study that was conducted at the American University of Beirut Medical Center , on patients undergoing laparoscopic gynecologic surgeries. This study was approved by AUBMCs Institutional Review Board and written informed consent was obtained from all patients. The study adheres to the CONSORT guidelines and was retrospectively registered at clinicaltrials.gov .

This study included female patients, aged between 18 and 60years, with American Society of Anesthesiologist physical status I or II scheduled for diagnostic or operative gynecological laparoscopic surgery of one to three-hour duration with abdominal incisions measuring less than 1.6cm in size. Patients with the following criteria were excluded: conversion of the surgery to laparotomy, requirement of an abdominal insufflation pressure greater than 14mmHg, history of gastro-esophageal reflux, thrombophilia or high risk of deep vein thrombosis according to the ACOG 2007 practice bulletin, pregnancy, morbid obesity , and 1 day surgery. Patients were randomly allocated to Groups 1 and 2 using a computer-generated randomization table. Blinding of the group allocation was not possible due to the design of the study.

How Do I Get Ready For A Cholecystectomy

Overview Of The Procedure
Laparoscopy is usually done under general anaesthesia. Generally, a small incision is made near the belly button and the abdomen is filled with CO2 gas. This lifts the abdomen away from the internal organs, giving the surgeon a better view. The lighted laparoscope is then inserted into the abdomen. Two other small incisions may be made in the abdomen so additional surgical instruments, such as a probe to move organs, can be used during the surgery. Many surgeons also attach a video camera to the laparoscope to enlarge the view, allowing the entire surgical team to see the procedure.
During the laparoscopy, the doctor examines the pelvic organs, looking for obvious and atypical endometriosis lesions as well as endometriomas , adhesions, and scarring. Depending on your history and symptoms, the doctor may also look for fibroid tumours or other abnormalities. Other procedures, such as a hysteroscopy may also be performed.
Diagnosis and treatment of endometriosis may take place during the same procedure. Your doctor may also remove the lesions to send to a lab for biopsy. This will document the presence or absence of endometriosis.
